Nurse Care Coordinator
A Nurse Care Coordinator (NCC) is a registered nurse who specializes in organizing patient care and treatments by including all members of the patient’s care team with the goal of improving patient outcomes, assisting patients in accessing the specialty care they need, preventing hospital and emergency room visits, and promoting a continuum of care for the patient . A nurse coordinator can assist in the creation of patient care plans and ensure that they are implemented according to the requirements of the facility. The NCC helps prevent gaps in patient care and assists in transitions of care.
A nurse coordinator usually has the responsibility of monitoring and organizing treatment for his or her designated unit or patients. This includes educating patients and their families, creating and implementing a care plan that meets the patient’s needs, reviewing the care plan and monitoring patient’s progress, maintaining patient medical records, communicating and coordinating with other direct care providers, researching additional healthcare resources and referring patients who need additional assistance.
